I had triple DVT's in both legs 30 years ago. Recently I had a rapid increase in the number of surface veins and phlebitis—worried.

I'm anti-coagulated with Warfarin and control my dosage at home with a blood test device. The initial DVT's were caused by bed rest after sciatica and a genetic protein S deficiency. I have been active working as a carpenter/cabinet maker. I went to ultrasound about six months ago and was told that the damage was consistent with my DVT diagnosis yet I am positive that I am experiencing a rapid deterioration in my condition over the last months. I am worried about quality of life/work/future.

For starters, wear compression stockings. The 10-20 mm/hg should be fine. If you are getting phlebitis (which are minor superficial blood clots), you may have an issue with your medication dosing, or you may have reflux of your superficial veins. A routine ultrasound will miss superficial reflux in many cases, so see a vein specialist.

You should be wearing a thigh length hose daily which may be problematic with your line of work but will certainly improve the superficial phlebitis you are experiencing. I would start with 20-30 mmHg and possibly a 30-40 mmHg if you can tolerate this. Staying well hydrated and exercising regularly, maintaining a sensible weight will all improve your future. If you haven't seen your vascular surgeon recently, perhaps a consultation would be beneficial.
Ensure your hose are correctly fitted early in the day and of a good quality for your comfort.
