How common is it to have to have a second microphlebectomy on a long ropey vein less than two months after the first procedure?

I had EVLA on both legs for varicose veins that have been present for over 18 years. I have also had a microphlebectomy done on a long ropey vein that went from top thigh down to lower calf. I am awaiting same procedure on other leg. One month after the procedure I am still seeing a significant amount of areas with raised veins. I contacted my physician and it was mentioned that I may have to have more pieces of veins removed. Is this common?

Varicose veins can be treated by Sclerotherapy or Phlebectomy. The former procedure almost invariably treats the entire very without leaving any untreated segments. Phlebectomy always removes small segments of the varicose vein and even with repeated treatments, there will be segments remaining behind.

While we try to remove as much as possible at the same time as the EVLA, there may be residual veins present. Often the residual veins will shrink down after the pressure from the saphenous vein is removed/sealed. If they are much smaller, they may be treated with sclerotherapy however some insurances will not cover this..

If this is the case then more than one microphlebectomy procedure may be required per leg. Sometimes, if a patient has an extensive amount of veins, I would just do the microphlebectomies in 2 stages.
